Herniation of extraocular muscles in the setting of orbital fracture is concerning for extraocular muscle entrapment, a clinical diagnosis.

Great review article on orbital emergencies: 

If you see fullness of the nasal septum in the setting of nasal trauma, bring up the possibility of nasal septal hematoma, which again is a clinical diagnosis. If left undrained, a nasal septal hematoma can lead to abscess or chronic nasal deformity.
