Major Findings

Left medial orbital wall blowout fracture
Medial rectus muscle partially enters the fracture defect
Comminuted nasal bone fracture with fullness of nasal septum

Teaching Points

Herniation of extraocular muscles in the setting of orbital fracture is concerning for extraocular muscle entrapment, a clinical diagnosis.

If you see fullness of the nasal septum in the setting of nasal trauma, bring up the possibility of nasal septal hematoma, which again is a clinical diagnosis. If left undrained, a nasal septal hematoma can lead to abscess or chronic nasal deformity.
