History
2 yo M with facial/orbital swelling
Major Findings
- Asymmetric right intraorbital fat stranding, consistent with post-septal orbital cellulitis
- Tiny subperiosteal abscess
- Extensive sinus disease
Teaching Points
Pre-septal = periorbital = may be managed with PO antibiotics
Post-septal = intraorbital = requires IV antibiotics
Complications to look for in orbital cellulitis:
- Subperiosteal abscess
- Intraorbital abscess
- Proptosis
- Superior ophthalmic vein thrombosis
- Intracranial spread of infection
