History
12 yo M with fever, eye swelling
Major Findings
- Right orbital pre-septal and post-septal cellulitis
- Right superior ophthalmic vein thrombosis
- Possible partial right cavernous sinus thrombosis
Teaching Points
Pre-septal = periorbital = may be managed with PO antibiotics
Post-septal = intraorbital = requires IV antibiotics
Complications to look for in orbital cellulitis:
- Subperiosteal abscess
- Intraorbital abscess
- Proptosis
- Superior ophthalmic vein thrombosis
- Intracranial spread of infection
