T-Bone Case 6

Expanded appearance of the right parietotemporal calvarial diploic space extending along the right greater sphenoid wing and right posterior zygomatic arch demonstrating heterogeneous enhancement and T2 FLAIR signal. Findings consistent with fibrous dysplasia.

Diffusion restricting, T1 hypointense, nonenhancing lesion centered in the mastoid temporal bone and middle ear measuring approximately 3.4 x 1.9 x 2.8 cm. It erodes mastoid osseous septations and extends through the posterior and superior external auditory canal walls with EAC effacement. Involvement and complete effacement of the right middle ear cavity with destruction of the ossicular chain.  Lesion extends through the lateral wall of the otic capsule with erosion of the lateral semicircular canal. Lesion abuts the lateral margin of the cochlea without definite associated invasion. Findings consistent with a cholesteatoma.
