Hypoattenuation and loss of gray-white matter differentiation in the right insula and portions of the temporoparietal lobes consistent with acute right MCA territory infarction. There is no significant midline shift or hemorrhagic transformation.
Discussion:
This case demonstrates the classic insular ribbon sign, with asymmetric hypoattenuation and loss of gray-white matter differentiation in the right insula, but with additional areas of hypoattenuation in the right temporal lobe consistent with additional areas of acute infarction. Remember to review your vascular territories; in this case, a rough approximation of various territories can be seen
