History
17 day old with Group B strep exposure, seizure
Major Findings
- Thrombosis of the superior sagittal sinus, likely thrombophlebitis related to meningitis
- Left hemispheric and right parietal infarctions, likely venous infarctions
- Effacement of sulci and basal cisterns, suggestive of cerebral edema, without herniation
Teaching Points
With extensive infarctions in the setting of infection, think of venous infarctions and look for hyperdensity in the venous sinuses to indicate clot.
Pearl: Fresh clot is high density on non-contrast CT and low density (filling defect, empty delta sign) on post-contrast CT.
Effacement of the sulci and CSF spaces can represent cerebral edema or even meningeal disease.
