Teaching Points

With extensive infarctions in the setting of infection, think of venous infarctions and look for hyperdensity in the venous sinuses to indicate clot.

Pearl: Fresh clot is high density on non-contrast CT and low density (filling defect, empty delta sign) on post-contrast CT.

Effacement of the sulci and CSF spaces can represent cerebral edema or even meningeal disease.
