Teaching Points
Intraventricular debris will layer dependently, usually in the posterior horns. Also look for periventricular edema as a sign of ventriculitis.
In the setting of meningitis, these deep parenchymal infarctions could be due to infectious angiitis or vasospasm.
A pertinent negative in this case is that there is no venous sinus thrombosis.
