History
3 yo M bit and run over by family dog
Major Findings
- Soft tissue injury in the left parietal/occipital scalp including scalp laceration and subcutaneous gas. Findings correspond with laceration behind the left ear seen on clinical exam.
- Depression and diastasis (widening) of the left occipitomastoid suture in the region of soft tissue injury. Compare the appearance of the occipitomastoid suture on the left with the right. This defect at the suture is likely due to a mildly dehiscent fracture secondary to a puncture wound from the canine's tooth.
Teaching Points
Use soft tissues to guide you to area of underlying calvarial fracture.
Use your 3D reformats of the skull to look for
suture lines and for fractures. With these reformats, remember to confirm
laterality as it may be reversed depending on which way the 3D skull is rotated.
